What Are the Current Types of Mesothelioma Surgery?

The current types of surgery for pleural mesothelioma include extrapleural pneumonectomy (EPP) and pleurectomy and decortation (P/D). A thoracotomy is the first part of an EPP or P/D surgery. This allows doctors to operate on organs inside the chest. Surgeons may also use a diagnostic procedure called video-assisted thoracoscopic surgery (VATS), which is inserting a tiny camera through a small cut in the chest to see the lung. VATS allows doctors to see inside the chest and take biopsy samples. The primary surgery for peritoneal mesothelioma is cytoreductive surgery, also known as debulking surgery.

Extrapleural Pneumonectomy (EPP)

EPP removes the cancerous lung and all nearby areas where mesothelioma spreads. This can include nearby lymph nodes, parts of the chest lining, heart lining and diaphragm.

Pleurectomy and Decortication (P/D)

P/D spares the lung and only removes cancerous parts of the pleural lining, the chest wall lining and the diaphragm.

Peritoneal mesothelioma is a form of cancer that forms in the peritoneum (the tissue lining of the abdomen). It is caused by inhaling or swallowing asbestos fibers. Patients with this condition may survive several years after diagnosis.

Tumors of this rare asbestos-related cancer form on the abdominal lining.

The beginning symptoms of peritoneal mesothelioma include abdominal distension, abdominal pain, swelling or tenderness around the abdomen and constipation or diarrhea.

The peritoneum is a protective membrane with two layers that surrounds the abdomen. Peritoneal mesothelioma can develop on both layers.

Parietal layer: Covers the abdominal cavity.

Visceral layer: Surrounds the stomach, liver and other organs of the abdomen.

Together, the layers support the abdominal cavity and its organs.

What Causes Peritoneal Mesothelioma?
Asbestos exposure is the primary cause of peritoneal mesothelioma, also known as abdominal mesothelioma.

Development of Peritoneal Mesothelioma
Swallowed asbestos fibers travel from digestive system to the peritoneum.

Inhaled asbestos fibers travel from lymphatic system to the peritoneum.

Once fibers are in the peritoneum, they irritate the cells.

The irritated cells begin to thicken the peritoneal lining.

Excess abdominal fluid builds.

Over time, tumors begin to form on the damaged peritoneum.

Research on other causes of abdominal mesothelioma is scarce. Evidence shows other fibrous minerals, such as erionite, and radiation to the abdomen trigger some cases of this disease.

Mesothelioma Stages

The stages of mesothelioma include stage 1 (cancer cells in one place), stage 2 (cancer cells entering lymph nodes), stage 3 (cancer cells spread to nearby organs and distant lymph nodes) and stage 4 (cancer cells spread throughout chest or abdominal cavity). Prognoses, survival rates and life expectancies decrease as stage number increases.

Cancer staging is a key part of mesothelioma diagnosis and treatment. Knowing how far the cancer has spread helps doctors decide which treatments to use. For example, doctors use cancer stage as a guideline when deciding whether a patient is likely to benefit from surgery.

Tumor size and location also determines whether a person can undergo surgery. Unfortunately, surgery is no longer an option when tumors become too big or spread too far.

Stage 1

Initially, tumor growth is within a single area. The life expectancy of stage 1 mesothelioma is significantly better than later stages. It is difficult to catch the cancer this early because people with this stage do not usually experience symptoms.

Major surgery to remove the cancer is the first line of treatment. Doctors usually recommend an aggressive multimodal approach including surgery, chemotherapy, radiation therapy or immunotherapy.

Median life expectancy at stage 1 is 22.2 months with surgery.

Stage 2

Stage 2 symptoms are vague and mild, and patients and doctors often mistake them for signs of other illnesses such as the flu.

The available treatment options are generally the same as for stage 1 mesothelioma. Median life expectancy at stage 2 is 20 months with surgery.

Stage 3

Once the cancer progresses to stage 3, it may have spread to several tissues, organs and lymph nodes in the same region of the body where it formed.

Pleural mesothelioma patients may have difficulty breathing and chest pain even when resting. Discomfort may be felt in other parts of the body as well.

Some stage 3 cancer patients are ineligible for surgery and other aggressive treatments. At that point, doctors offer palliative options.

Median life expectancy at stage 3 is 17.9 months with surgery.

Stage 4

By stage 4, cancer has spread to other parts of the body. Cancer could be present in the liver, brain, bones or elsewhere. Patients may experience extreme difficulty breathing and swallowing, digestive problems and severe pain.

For stage 4 patients, doctors typically fall back on palliative treatments designed to ease pain and control other symptoms. Most stage 4 patients do not qualify for surgery.

Median life expectancy at stage 4 is 14.9 months or less with or without surgery.

Texas Asbestos Laws and Regulations

The Texas Department of State Health Services regulates most of the asbestos laws for the state under the Texas Asbestos Health Protection rules and regulations, which closely follows federal standards.

Training and Licensing of Asbestos Professionals in Texas

Texans handling asbestos must be trained, licensed, and certified within the state. Out-of-state applicants who completed their training and certification outside of Texas must at least undergo three hours of hands-on training by a licensed provider in Texas. In general, training courses are valid for one year and will require annual refreshers. Training consists of an 8-hour day broken up by lectures, demonstrations, and hands-on work, though courses will differ based on the specialty. Individuals will then need to pass a state licensing exam based on their training, and can only retake the test twice upon failing. Further regulations are specified in Chapter 295 of Title 25 of the Texas Administrative Code.

Texas has a statute of limitations of just 2 years for both personal injury and wrongful death claims. After seeing such a high-volume of asbestos-related lawsuits, Texas began to tighten the regulations for claimants. Originally, the laws cited under Title 4, Chapter 90 of the Texas Civil Practice and Remedies Code required claimants to have documented proof of their diagnosis, proof exposure occurred at least 10 years ago, as well as a medical and smoking history. In 2007, however, the Texas Supreme Court added a rule that claimants also had to prove the level of asbestos exposure they faced. While lawmakers contested this decision, it was upheld in a 2014 ruling. Many asbestos lawsuits today are consolidated into multidistrict litigations (MDLs).
